Heres the form, short, I know, but it has everything needed!
Name~ (your profile-name ex. Music-Luver, Firenza)
Surprise?~ (if you want me to add somthing that you didnt state)
Writing~ (writing (if you have any in mind) that you want on the graphic)
Comments~ (anything you want me to add on, ex. stars, faded)
Pictures~ (if you specifically want, or suggest some pictures for the graphic (prefered in link format) it can be more than one)
